Major Services such as crowns, oral surgery, periodontics covered at 50% after one year
Annual Deductible on Basic & Major services $50 per person or $150 per family
Increasing Calendar Year Maximum: 1st Year = $750, 2nd Year = $1,000, 3rd Year+ = $1,500
